REGISTRATION PROCEDURE

1. Download the registration form. Complete / Fill in the form.
2. Choose your preferred conference package and appropriate conference fees. Please be guided by the following fee schedule.

3. You can pay your conference fee either through bank deposit or credit card.
   1. Bank Payment

      Deposit your payment to the accredited bank below.

      Local attendees can deposit their payments to:

      Fax the deposit slip and completed registration form to +63 49 536-6013 or email scanned copies of the same to icodelsecretariat@upou.edu.ph.

   2. Credit Card Payment

      Fill out the UPOUFI Credit Card Payment Authorization Slip. Note that the amount that will be billed to your credit card will be in Philippine Peso (PhP) subject to existing foreign exchange rate.

      Fax the completed registration form and signed copy of the authorization slip to +63 49 536-6013 or email scanned copies of the same to icodelsecretariat@upou.edu.ph.

      An Official Receipt issued by UP Open University Foundation (UPOUFI) will be sent to your email account to confirm your payment.
